I. Identification – complete your info. For “Firm/Agency” just put “SELF.”
II. Representation = “SELF.”
III. POSITION = PROPONENT
IV. TESTIMONY = Record of Appearance ONLY
Check the box at the bottom “I agree to the Terms of the Agreement”
Click the “Create(Slip)” button & you are done.
